Tirzepatide belongs to a class of medications known as GIP/GLP-1 receptor agonists. It's a first-in-class dual agonist, meaning it activates two separate gut hormone receptors — GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) — rather than just one, as older diabetes and weight-loss medications typically do.

Compounded tirzepatide is custom-prepared by individual pharmacies rather than mass-manufactured by Lilly, and it isn't FDA-approved as a finished product. It became widely available during the 2022–2024 Mounjaro/Zepbound shortage, when compounding rules were temporarily relaxed. Since the FDA declared that shortage resolved in late 2024, compounding is now permitted and the legal rules are still evolving.
